Use this form to notify Victoria University if you test positive for COVID-19.

The information in this form is required under the Diagnosed Persons and Close Contact Direction, issued by the Victorian Government.

Any person, staff or student who is diagnosed with COVID-19 must inform the University of their infection. This is regardless as to whether you are working on campus or off campus.

On-campus incidents will be assessed and it is now the requirement on the University to contact any person who may have been exposed to COVID-19. Each person that is potentially exposed will be notified of this potential exposure.

Off-campus notifications assist in ensuring appropriate actions can be undertaken for future exposures.
