Overview

This unit describes the performance outcomes, skills and knowledge required to perform nursing interventions to support the social and emotional wellbeing and nursing care needs of the older person in care.

This unit applies to enrolled nursing work carried out in consultation and collaboration with registered nurses, and under supervisory arrangements aligned to the Nursing and Midwifery Board of Australia (NMBA) regulatory authority legislative requirements.

Assessment

For Melbourne campuses

Assessment tasks will be designed to reinforce & extend knowledge and skill competence within set & controlled parameters in accordance with each unit’s learning outcomes & performance criteria requirements, including the setting of work based practical application tasks designed to provide evidence of competence outcomes, within periodic and scheduled timelines.

Students will be expected to demonstrate the following required skills:
• analyse health information and clinical presentation of two older people to reach a conclusion on possible nursing interventions related to their care in consultation with a registered nurse
• perform nursing interventions for one older person in the workplace with cognitive change or impaired cognition and one older person with the condition of frailty

The tasks must be undertaken in accordance with the Nursing and Midwifery Board of Australia (NMBA) professional practice standards, codes and guidelines.

Students will also be expected to demonstrate the following knowledge:
• role of aged care regulators.
• the potential impacts of dual and multiple diagnoses on identification and prioritisation of the older person’s needs
• chronic psychological and age-related health problems
• sexuality and the older person
• personal hygiene:
• activities of daily living (ADLs)
• continence management
• oral care
• counselling and support services and resources in the aged care environment and wider community
• support and comfort required for grieving families and carers within own role parameters
• difference between end-of-life care and palliative care of dying and after death care in consultation with registered nurse
• functional assessment tools for assessing older people, including recognition of cognitive impairment including delirium and depression
• nature of cognitive change related to progressive neurological condition and strategies and nursing interventions that can be used
• diagnoses of dementia and how it presents
• mini mental and psychogeriatric assessment scale using a developed and recognised standard assessment scale
• the importance of a safe, physical, social and emotional environment for older people, and particularly for people with cognitive impairment
• legal and professional obligations relating to physical, chemical and psychological restraint
• legal and ethical issues and considerations, and assessment tools relevant to the older person:
• advance care directives
• Aged Care Act
• Aged Care Funding instruments
• Aged Care Quality Standards
• carers recognition legislation
• NMBA code of ethics and code of conduct for nurses
• primary health care and services for the older person
• stereotypes and influences on ageing
• physical triggers and strategies to respond to responsive behaviour:
• distraction including reminiscing, walking and listening to music
• behaviour modification
• pain management
• use of prescribed hearing aids and alternative listening devices
• integration of complementary therapies into practice settings
• elder abuse
• nursing assessments of the older person:
• nutritional assessment
• skin assessment
• continence assessment
• physiology of the ageing process as it relates to disease processes:
• sensory function
• cardiac function
• respiratory function
• neurological function
• musculoskeletal function
• genitourinary function
• endocrine function
• renal function
• oncological disorders
• integumentary functions.

Required reading

The qualified trainer and assessor will provide teaching and learning materials as required in the form of workbooks produced by the VUPolytechnic and/or via the VU Polytechnic e-learning system.
