For any screening questions you answer 'Yes', please provide details where relevant. 

This may include the condition or injury, date of diagnosis or onset, current symptoms, treating practitioner, treatment or management plan, medications, restrictions or recommendations relating to exercise. Include conditions that are well controlled, stable, previously treated, resolved or currently in remission.
