Overview

This unit describes the skills and knowledge required to contribute to the care of a person with chronic health problems by performing nursing interventions that support the person’s needs and assist them in maintaining an optimal lifestyle. This unit applies to enrolled nursing work carried out in consultation and collaboration with registered nurses, and under supervisory arrangements aligned to the Nursing and Midwifery Board of Australia regulatory authority legislative requirements.

The skills in this unit must be applied in accordance with Commonwealth and State/Territory legislation, Australian/New Zealand standards and industry codes of practice.

Assessment

For Melbourne campuses

Assessment tasks will be designed to reinforce and extend knowledge and skill competence within set and controlled parameters in accordance with each unit’s learning outcomes and performance criteria requirements, including the setting of work based practical application tasks designed to provide evidence of competence outcomes, within periodic and scheduled timelines.

Students will be expected to demonstrate the following required skills:

undertaken nursing work in accordance with Nursing and Midwifery Board of Australia professional practice standards, codes and guidelines;
analysed health information and the clinical presentation of 1 person in the workplace or simulated environment to reach an accurate conclusion on possible nursing interventions related to their chronic health problems, in consultation with a registered nurse, and;
performed nursing interventions in the workplace to contribute to the support of 2 people who have chronic health problems.

Students will also be expected to demonstrate the following knowledge:

broad context for chronic disease;
rehabilitation strategies, techniques and equipment used for chronic health conditions;
clinical manifestations of chronic disease or chronic health conditions, and;
strategies to address and manage chronic disease in the relevant work context.

Required reading

The qualified trainer and assessor will provide teaching and learning materials as required in the form of workbooks produced by the Polytechnic and/or via the Polytechnic e-learning system.
